Company President - Tyrone S. Tonkinson, Ph.D., P.E.
SAI instructional material is developed from proven theories and Ty’s first-hand experiences from hundreds of investigations and organizational assessments. Ty has led team assessments of up to 20 people and performed numerous root cause analysis investigations and process audits. Ty was a featured speaker at the Annual Human Performance / Root Cause / Trending (HPRCT) Conferences in 2002, 03, 04, 07, and 08. Ty is also a published author. His presentation on "Behavior-based Corrective Actions for Results" was published in the transcripts of the 50th anniversary American Nuclear Society conference in Pittsburgh in June 2004. Incident Prevention magazine published his two-part article on interviewing techniques, and also a separate article on managing safety rule violations. Ty has been teaching advanced root cause analysis and organizational assessment techniques since 1991.
